Recently several visitors of websites have asked me about this ebook, which is being advertised quite widely across the Internet. So I purchased a copy myself to figure out what all the publicity was about.

Overall my first impression of this book is good. I think it was sincerely written and looks to be very helpful.

Human Abnormalities Deformities

Anal skin tags are small outgrowths of the mucosa arising from the anal margin. They are relatively common and have no pathologic significance. Figure 2.81. Preauricular skin tags are extremely common, but the presence of skin tags between the ear and comer of the mouth would suggest a diagnosis of Goldenhar's syndrome. Figure 1.74. Cebocephaly

This infant with Goldenhar's syndrome has unilateral macrostomia. There was an antimongoloid slant to the eyes, preauricular skin tags, and deafness. Figure 1.72. Macrostomia with cutaneous tags and preauricular skin tags. Cutaneous pits and tags may be found along a line connecting the oral commissure and the external auditory canal in Goldenhar's syndrome.

No risk during pregnancy has been identified for the virostatic acyclovir (see also extensive experience with sytemic use during pregnancy, Chapter 2.6) or for local treatment for condylomata acuminata (venereal warts) with podophyllotoxin, a plant-based mitosis inhibitor (Robert 1994, Bargman 1988, Kami 1980).

Types Branchial Cleft Skin Tag

Goldenhar's syndrome (facio-auriculovertebral spectrum oculoauriculoverte-bral dysplasia) is associated with abnormalities of the first and second branchial arches. This infant shows the antimongoloid slant, bilateral macrostomia, and skin tags. Over 90 of these infants have ear abnormalities (small or unusually shaped ears, preauricular tags, and pits). They may have abnormalities of the cervical vertebrae, particularly hemivertebra, coloboma of the upper eyelids, and epibulbar dermoids. Congenital heart disease may be present in one-third of these infants. More than 80 of the infants have normal intelligence. Figure 3.59. Another infant with Goldenhar's syndrome showing the abnormal ear and preauricular skin tags in a line extending from the ear to the macrostomic mouth. Characteristic of Goldenhar's syndrome is the combination of unilateral facial hypoplasia, epibulbar dermoid, ocular abnormalities, preauricular appendages, and unilateral dysplasia of the auricle.

Fetal Risk Summary

A study published in 1995 assessed the risk of congenital malformations in infants of mothers with non-insulin-dependent diabetes during a 6-year period (21). Women were included in the study if, during the first 8 weeks of pregnancy, they had not participated in a preconception care program and then had been treated either with diet alone (group 1), diet and oral hypoglycemic agents (predominantly chlorpropamide, glyburide, or glipizide) (group 2), or diet and exogenous insulin (group 3). The 302 women eligible for analysis gave birth to 332 infants (five sets of twins and 16 with two or three separate singleton pregnancies during the study period). A total of 56 (16.9 ) of the infants had one or more congenital malformations, 39 (11.7 ) of which were classified as major anomalies (defined as those that were either lethal, caused significant morbidity, or required surgical repair). The major anomalies were divided among those involving the central nervous system, face, heart and...

Genital or veneral warts are condylomata acuminata caused by the human papilloma virus (HPV) of several types, especially 6 and 11. Treatment is usually by electrocautery, laser, cryotherapy, or surgical excision. Chemical abalation can be used, and involves the use of topical antimitotics such as podophyllotoxin, podophyllin, or 5-fluorouracil, or caustics such as trichloroacetic acid. Interferon-inducers such as imiquimode are also used, and again require multiple applications. Information about these chemicals while breastfeeding is not available, but both the area treated and the dosage are small. Interferon is known to be a large molecule that does not pass into milk even during systemic treatment. The development of the HPV vaccine has shown a decrease in the incidence of these lesions, and will perhaps decrease the need for these medications.

Hyperinsulinemic insulin resistance

Clinical findings that suggest the presence of insulin resistance and hyperinsulinemia include body mass index (BMI) 27 kg m2, waist-to-hip ratio 0.85, waist 100 cm, acanthosis nigricans and numerous achrochordons (skin tags).56 However, according to the American Diabetes Association (ADA) Consensus Conference,57 there is still no satisfactory method for determining insulin resistance in the clinical practice setting. None of the tests, such as fasting insulin, glucose or glucose-to-insulin ratio, has been shown to be a useful predictor of the ovulatory response to insulin-sensitizing drugs. Although the fasting glucose-to-insulin ratio (

Butthole Tufts Hair

A midline hair tuft in the lum-bosacral area. This infant had a tethered cord on MRI study. Hair tufts, skin tags, sinuses, and abnormal pigmentation that occur in the midline along the length of the spinal column should always alert one to the possibility of an associated underlying neurologic abnormality. With a tethered cord the neural tissue is firmly attached at its caudal end, being bound by a stout connective tissue band to the interior of the bony canal. With growth, the spinal canal normally grows more rapidly than the spinal cord resulting in traction on the cord. This may gradually pull the lower end of the brainstem down into the foramen magnum like a cork into a bottle. This is the Arnold-Chiari malformation. Figure 3.58. A midline hair tuft in the lum-bosacral area. This infant had a tethered cord on MRI study. Molluscum Contagiosum

Symptoms Small (two to five millimeters), dome-shaped skin bumps that are flesh-colored, smooth, and sometimes shiny in appearance occur with this infection. The lesions are harmless, involve only the skin, and resemble warts (but are not true warts). The lesions tend to occur most commonly in school-aged children and are generally located in small clusters (10 lesions or fewer) on one part of the body, usually on the face, neck, hands, or lower legs. They do not cause discomfort, except for mild itching. Occasionally, the lesions can be spread to other parts of the body by scratching (autoinoculation), and they may look cos-metically unappealing, especially if located on the face or neck. They sometimes also

Preauricular Tags Newborns

Preauricular skin tag in an otherwise normal infant. These skin tags often contain a core of cartilage and appear to represent accessory hillocks of His. Hillocks normally develop in the recess of the mandibular and hyoid arches and coalesce to form the auricle. Figure 1.143. Preauricular skin tag in an otherwise normal infant. Hillocks normally develop in the recess of the mandibular and hyoid arches and coalesce to form the auricle.

21713 5Fluorouracil

Dermal treatment with 5-fluorouracil is contraindicated in pregnancy, apart from for the treatment of an individual wart. Treatment of vaginal warts should be postponed until after the birth, or other treatments (such as cryotherapy) used instead. Local use of the cytostatic is, however, not an indication for interruption of pregnancy or for invasive diagnostic procedures.

Exstrophy Cloaca Sequence

Another example of the exstrophy of the cloaca sequence. Note the exstrophy of the cloaca and hydromelia. This infant also developed an in utero amputation of die right lower extremity, probably from an amniotic band and had a gangrenous skin tag on die buttock. The left foot shows severe talipes equinovarus. Figure 4.135. Another example of the exstrophy of the cloaca sequence. Note the exstrophy of the cloaca and hydromelia.
