Fetal Risk Summary

A 1989 case report described the effect of maternal chemotherapy on a premature newborn delivered at approximately 27 weeks' gestation (6). The mother was treated with bleomycin (30 mg), etoposide (165 mg), and cisplatin (55 mg) (all given daily for 3 days), 1 week before delivery, for an unknown primary cancer with metastases to the eye and liver. The mother developed profound neutropenia just before delivery. On the 3rd day after delivery, the 1190-g female infant also developed a profound leukopenia with neutropenia, 10 days after in utero exposure to the antineoplastic agents. The condition resolved after 10 days. At 10 days of age, the infant began losing her scalp hair and experienced a rapid loss of lanugo (6). Etoposide was thought to be the most likely cause of the neutropenia and the alopecia (6). Other Dermatologic Problems

The typical facies of hypohidrotic (anhidrotic) ectodermal dysplasia is seen in this infant. Note the alopecia, absent eyebrows and eyelashes, square forehead with frontal bossing, hyperpigmented wrinkles around the eyes, flattened nasal bridge, and large conspicuous nostrils. There are wide cheek bones with depressed cheeks, thick everted lips, a prominent chin, and the ears may be small and pointed. These infants have a thin dry skin, decreased sweating, decreased tearing, and abnormal dentition. The nails are defective in a large percentage of these patients in that they may be thin, brittle, or ridged. If the absence of the sweat glands is generalized, they may have recurrent fever in high environmental temperatures.

Transient pancytopenia was reported in some children following exposure during the second third trimester (Hsu 1995, Murray 1994). One case report describes a premature baby that developed cerebral atrophy with enlargement of the cerebral ventricles its mother had been treated for an ovarian tumor and received 100mg m2 etoposide for 5 days in week 26 27 in combination with bleomycin and cisplatin (Flit 1999). Re-examination after a year showed normal neurodevelopmental progress, but there was moderate sensorineural hearing loss bilaterally (Rattles 1989).

921Anorexia Nervosa

Characterized by extreme voluntary weight loss due to self-starvation or binge eating followed by purging, AN occurs in 0.5-3 of the female population 3, 4 . Clinical signs and symptoms of AN include an emaciated appearance, prepubertal features, lethargy, lanugo, alopecia, acrocyanosis, hypothermia, swollen joints, pitting edema, and bradycardia and hypotension. Biochemical evaluation often shows fluid and electrolyte disturbances and hypercarotenemia as well as endocrine and hematologic abnormalities such as hypothyroidism and anemia, respectively. Several cardiovascular irregularities develop along with a host of gastrointestinal complications, particularly in those with the binge eating-purging type of AN. Osteoporosis and skeletal fractures are common in persons with AN. Some may experience peripheral neuropathy and seizures. Mortality is as high as 22 in women with long-term AN 5 .

Minoxidil is used orally as a systemic antihypertensive drug with vasodilatory activity. Topically, it is used in cases of androgenetic alopecia and other types of baldness. The ingredient is lipophylic, and the resorption rate is 2-3 o. The concentrations in the serum are far below therapeutic levels in adults. In a prospective study, 17 pregnant women were treated with minoxidil. Of 15 newborns, 1 had an unspecified developmental disorder of the heart (Shapiro 2003). A case report (Smorlcsi 2003) describes a woman who applied minoxidil to her scalp at least twice daily during pregnancy. Fetal pathology revealed an enlarged heart with distal stenosis of the aorta, an enlarged sigmoid colon and mesentery, and enlarged cerebral ventricles with brain hemorrhages. A case of chronic intoxication by a rodenticidc containing thallium at the workplace has also recently been described. Most of the children survived the poisoning of their mother, if she was treated adequately. It has been determined, primarily from animal studies, that there is a species-dependent constant brain-to-blood ratio for mercury that is 5 1 in primates. Scalp hair is an excellent indicator of blood levels and can be used to assess individual exposures to mercury. From studies done by Clarkson and colleagues during the Iraqi outbreak, a threshold in the adult response was determined to be 50 to 100 mg Hg per gram of hair the prenatal threshold was 10 to 20 mg Hg per gram of hair, indicating that the fetus may be five to ten times more sensitive to the effects of methylmercury than the adult.

Symptoms The infection starts as a round, reddish sore almost anywhere on the child's body. As the fungus grows, it spreads outward in all directions, causing the spot to become larger.
